Understanding Body Mass Index (BMI)

Body Mass Index (BMI) is a simple numerical scale used by healthcare practitioners to estimate whether an individual is at a healthy weight for their height. Originally formulated in the 19th century by the Belgian statistician Adolphe Quetelet (known as the Quetelet Index), BMI categorizes adults as underweight, normal weight, overweight, or obese.

Clinically, BMI correlates with total body fat at a population level. It is used as a screening tool to identify possible weight problems. However, it is not diagnostic: an individual with a high BMI should undergo further health assessments (such as body fat calipers, skinfold measurements, waist circumference checks, and cardiovascular screenings) to assess overall wellness.

Standard BMI vs. New BMI (Trefethen Formula)

Traditional BMI is calculated as:

BMI = Weight (kg) / Height (m)2

In 2013, mathematician Nick Trefethen of Oxford University pointed out a fundamental flaw: traditional BMI assumes human beings scale uniformly in two dimensions. However, taller people are not just stretched versions of shorter people—they have more volumetric mass. Traditional BMI therefore overestimates weight status for taller people and underestimates it for shorter people.

Trefethen proposed the New BMI formula:

New BMI = 1.3 × Weight (kg) / Height (m)2.5

The 1.3 multiplier is a scaling factor chosen so that the New BMI matches standard BMI exactly for a person of average height (1.69 meters, or roughly 5 feet 7 inches). Shorter people will find their New BMI score is slightly higher, while taller individuals will see a slightly lower, more accurate rating.

The "Bodybuilder Paradox" & Muscle Mass

Because BMI depends only on total weight and height, it does not differentiate between fat tissue and lean muscle tissue. Muscle is roughly 18% denser than fat. Consequently, weightlifters, bodybuilders, crossfitters, and highly muscular athletes frequently score in the "Overweight" or even "Obese" categories of traditional BMI tables.

To resolve this discrepancy, it is crucial to measure your body fat percentage and look at your Fat-Free Mass Index (FFMI). An FFMI above 21 for men and 18 for women indicates high muscularity. If you have low body fat, a high BMI is not a sign of poor health.
